Your appointment

On this page you should find everything you need to know before your your appointment. 

The service has steps in place to ensure that it is safe when you attend in person. These measures include socially-distanced waiting areas, hand sanitisers and members of the team wearing personal protective equipment e.g. masks / visors.

To help us please: 

  • Bring appropriate clothing, such as shorts or vest top, as you may need to have a physical examination of your affected body part.
  • DO NOT attend if you or anyone you have been in close contact with has had any Covid symptoms. ie Continuous cough, high temperature, loss of taste or smell
  • DO NOT attend if any symptoms develop prior to appointment date
  • Please arrive on time, not early and inform reception of your arrival

If you have developed any COVID symptoms outlined above prior to your appointment date, please contact us as we still may be able to offer you a telephone or video consultation as an alternative to attending your appointment.

 

Please read this document carefully before your video appointment.

After your appointment you may be offered a Patient Initiated Follow Up (PIFU). This will be decided after a discussion between you and your clinician.

PIFU is a type of open appointment that gives patients and their carers the flexibility to arrange their own follow up appointment whenever they need it, during a three-month period from your last appointment.

If your symptoms flare up or your circumstances change within your agreed PIFU period, you may book a follow up appointment with your clinician by calling 01246 565050 or by completing our online contact form.

Airmid is a new health app that helps you manage your appointments and communications with Derbyshire Community Health Services NHS Foundation Trust.  Many of our services are starting to offer Airmid as a new way of communicating with our patients.
